Hi all,

Hoping someone can help me here. I teach beginning lampworking classes and I think it would be nice to offer the students an evaluation form they can fill out, evaluating my class and their experience.

Does anyone know if there is already a form available out there for instructors to use, or should I just make my own?

Dave gave you some good ideas for starting points. Besides tailoring something to be more specific to glassworking, I'd suggest thinking about what specific kinds of things would you like to know from your students - what information would help you grow as a teacher and be able to attract new students. Remember to add some lines at the bottom for "Other Feedback/Suggestions" for things they think of that you might not have thought to ask. The more pertinent to glassworking, your studio, your material, your style, etc, the more they'll be interested in filling it out and giving you thoughtful, candid feedback. Whenever I see a vanilla, generic questionaire, I tend to blow it off, but if it looks to me like someone really thought about the questions, I'm more inclined to take them seriously and respond.

A suggestion for you is to hand the questionaire's out at the beginning of the class with whatever printed material you provide and briefly go over it. Spend less than a minute, but briefly explain what you're trying to get out of the questionaire and ask them for their ideas for making your classes even better (great hook for encouraging them to come back or recommend you to their friends). Most folks will naturally look at the questions and have them in the back of their minds as they take the class. You'll probably get better feedback than if you wait until the end (when they all want to rush home).

I wound up making my own, after giving it much thought as to what I wanted on it, but these suggestions are awesome. I can always adapt and make changes to my form, if I need to.

I think I covered most of my bases, though.
